. A passport or driving licence . A current utility bill, bank or credit card statement (dated in the last 3 months) . Your 3 most recent payslips or social welfare receipts . Proof of any other household income . Your bank statements for the past 3 months

Self employed applicants must prove that their tax affairs are in order and up to date by providing their most recent Notice of Revenue Assessment or a copy of their Form 11.
